Foreign research
There is a lot of research on robot assisted positioning and navigation accuracy abroad. Pollock et al. in the United States used artificial models to study computer-assisted navigation systems, and the average error is (5.8 ± 1.2) mm (1.8-11.9mm); Oliveira et al. conducted research on percutaneous puncture, and found that the average error in bone tissue was 4.23mm, while the average error in soft tissue was 3.07mm; The navigation system has a large error, which is mainly caused by such influencing factors as image registration, positioning technology, matching algorithm and image drift. The most important influencing factor is image drift, with an incidence of up to 66%. In order to overcome the error caused by image drift, an embedded robot system has emerged in the market, which embeds robots into CT equipment, such as Acubot robotic surgery system, which is applying for PDA certification. Although this system greatly improves navigation accuracy, it has a significant radiation impact.
The Renaissance robot manufactured by Israel's Mazor Robotics Co., Ltd. is applicable to brain tissue biopsy and extended to cervical spine surgery. The accuracy of nail placement reaches 98.3%, the axial deviation is (1.2 ± 1.49) mm, the sagittal plane deviation is (1.1 ± 1.15) mm, and the overall accuracy is controlled within 3mm. A study by Koethe et al. in the United States found that under CT guidance, robot assisted abdominal puncture was performed, and the distance between the puncture point and the target point was verified through a model to be (6.5 ± 2.5) mm vs (15.8 ± 9.2) mm (barehanded puncture), with statistical differences in the results. Barzilay et al. performed lumbar fusion for 19 patients, and 9 of them had clinical errors, which was greatly related to image registration and robot arm loosening.
Domestic research
Robot positioning navigation assisted surgery has developed very rapidly in China, and neurosurgery department of the PLA General Hospital has used French Rosa robot to implant electrodes in more than 1000 patients with epilepsy. From August to October 2015, Beijing Jishuitan Hospital successively completed multiple intraoperative real-time 3D imaging robots assisted internal fixation surgery for thoracolumbar fractures, with a positioning accuracy of less than 2mm. Although the robot is used in orthopaedics and neurosurgery with good results, it is difficult to puncture abdominal organs. Wang Wendong and others used the robot to puncture pig liver, and found that the average range of pig liver deformation during the puncture process was 1mm, and the difficulty of puncture in flexible media increased.
Wang Yang et al. applied navigation and robot technology to ultrasound guided thermal ablation therapy of liver tumors, and found in animal experiments that the error between the microwave needle and the target point in the liver was (4.48 ± 0.95) mm; Zhang Xiao applied minimally invasive real-time robot navigation to rabbit liver tumor puncture, with a single needle error of (4.84 ± 0.99) mm and a double needle error of (6.20 ± 1.21) mm. Compared to rigid static organs (intracranial or skeletal), robots have a greater error in puncturing abdominal organs, which is closely related to the displacement of target points caused by respiratory movements and liver deformation caused by the puncture process. Further research on gating methods is needed to reduce these effects.
